Glenbrook Rehabilitation and Skilled Nursing Center received a deficiency-free survey rating during an annual inspection by the Indiana State Department of Health.
A deficiency-free score is the best score a facility can receive and is a distinction achieved by a low percentage of facilities in the state.
The health department inspects annually to assess compliance with state standards of care, such as adequacy of staffing, quality of care and cleanliness of facilities.
Located at 3811 Parnell Avenue, Fort Wayne, the senior health and rehabilitation community is operated by American Senior Communities.
ASC Chief Operating Officer Dan Benson said Glenbrook Rehabilitation and Skilled Nursing Center’s staff operates by the company’s core values.
