Bethlehem Woods Nursing and Rehabilitation was found to have no deficiencies during an inspection by the Indiana State Department of Health. This is the second year the community has received this rating.
Operated by American Senior Communities, Bethlehem Woods is located at 4430 Elsdale Drive in Fort Wayne. The State Department of Health inspects it and all other communities to assess compliance with federal and state standards of care, such as adequacy of staffing, quality of care and cleanliness of facilities.
A deficiency-free score is the best score the facility can receive and is a distinction achieved by a low percentage of facilities in the state. Receiving two consecutive deficiency-free ratings is outstanding.
“Compassion, Attentiveness, Relationships and Excellence are the core values for American Senior Communities. These words not only form an acronym for C.A.R.E, they are guiding principles that lead us every day as we serve a very special generation,’’ said Dan Benson, chief operating officer of American Senior Communities.
“We are so proud of the staff for living out these values in an exceptional way. We congratulate the staff of Bethlehem Woods for their perfect survey from the Indiana State Department of Health. Without the staff’s high level of dedication and commitment, a deficiency-free survey would not be possible.”
Bethlehem Woods offers Moving Forward Rehabilitation, Auguste’s Cottage Memory Care, Hospice, Respite, Skilled and Long-Term Care. For more information, call the community at 260-485-8157, or visit the website at www.ascseniorcare.com/bhw
